You may drop off completed packets or pick up packets to complete at any Dowagiac Elementary School on these days/times
May 4, 2020 from 9:00 a.m. to 3:00 p.m.
May 11, 2020 from 9:00 a.m. to 3:00 p.m.
May 12, 2020 from 12 p.m. to 6:00 p.m.
May 26, 2020 from 12 p.m. to 6:00 p.m.
WHO: For parents of children who will be five years old on or before
September 1, 2020.
WHAT YOU WILL NEED:
1. Student’s Birth Certificate
2. Current Immunization Records
3. One form of Verification of Residency (i.e. Electric Bill, Cell Bill)
4. Driver’s License
